Exploring sleep problems in children with developmental disabilities, more here..

;
Existing studies that have identified an increased risk of sleep problems in children with intellectual disabilities have often included, in one group, children with neurodevelopmental disorders caused by different genetic syndromes or social and communication disorders.

Image may contain: 1 person, standing

The type of neurodevelopmental disorder may be important in the study of sleep disturbance. This is the case for other behavioral and social communication difficulties, as the profiles of these difficulties, such as the combination of problems and the proportion of children they affect differ between neurodevelopmental disorder groups according to a previous study by researchers from the Cerebra Centre for Neurodevelopmental Disorders.
We sought to build on previous research in a new study, funded by Cerebra and published in Journal of Neurodevelopmental Disorders. This used parent questionnaires to identify whether children with different neurodevelopmental disorders (such as Angelman syndrome, Smith-Magenis syndrome, Tuberous Sclerosis Complex, or autism) differ in the severity and combination of sleep problems when compared to children without a neurodevelopmental disorder. We looked to identify whether the cause of a child’s neurodevelopmental disorder (genetic or not) may be an important determinant in the symptoms of their disturbed sleep.
We found that sleep problems occur more frequently and more consistently in certain neurodevelopmental disorder groups.
We found that sleep problems occur more frequently and more consistently in certain neurodevelopmental disorder groups. For example, at least 73% of children with Smith-Magenis syndrome experience severe night and/or early morning waking. In contrast, a smaller proportion of children with Angelman syndrome (46%) and autism spectrum disorder (43%) experienced severe night waking problems.
Whilst severe sleep problems do not affect all children with Angelman syndrome, autism spectrum disorder or tuberous sclerosis complex, these children scored higher on many subscales of the questionnaire when the findings were compared to data from typically developing children:
  • Children with Smith-Magenis syndrome, Angelman syndrome, autism spectrum disorder and tuberous sclerosis complex had higher scores on the night waking, parasomnias (abnormal behavior during sleep), and daytime sleepiness subscales of the questionnaire.
  • Children with autism spectrum disorder predominantly experience difficulties with night waking and falling asleep.
  • Children with Smith-Magenis syndrome primarily experience night waking, early morning waking problems, and behavioral symptoms of sleep disordered breathing.
  • Children with Angelman syndrome primarily experience night waking difficulties, followed by early waking problems and behavioral symptoms of sleep disordered breathing.
There was also an association between sleep disturbance and existing painful health conditions for the Angelman syndrome and tuberous sclerosis complex groups. A syndrome-specific approach to assessing and treating sleep disturbance could therefore include assessing children for painful health conditions. This example highlights how known health and behavioral phenotypes of different neurodevelopmental disorder groups may present added risk for poor sleep.
The authors also found different associations with over activity and impulsivity, health, and gastro-oesophageal reflux with sleep disturbance across the groups. This suggests that while children with diagnosed neurodevelopmental disorder are general more vulnerable to sleep disturbance, specific profiles of sleep problems are evidenced for different neurodevelopmental disorder groups.
Therefore, we found that the cause of the neurodevelopmental disorder and its associated phenotype may have specific mechanisms which underpin sleep disturbance in each group.

Mesothelioma Cancer Centers, the specific areas..

;
Anyone whose life has been touched by mesothelioma understands that it is necessary to locate the best care available for this rare form of cancer which often proves difficult to treat. Finding excellent doctors and other medical professionals who are noted experts in their field and who specialize in mesothelioma is essential to ensuring the most favorable outcome.

Image may contain: 1 person, standing, phone and selfie
Most of the major cancer centers and clinics in the United States that are devoted specifically to mesothelioma care offer the latest and most advanced treatment options available for managing the disease. In addition, these centers employ physicians who are involved in the latest research and who stay abreast of new and emerging treatment protocols. The medical approaches of these physicians provide the best opportunities for extending a patient’s life expectancy and improving overall quality of life during the fight against asbestos-caused cancer.
Finding a Mesothelioma Clinic/Cancer Center
Those who live in large cities are often fortunate enough to have a major cancer center located in their proverbial backyard. Many of these cancer centers can be found at the nation’s leading universities. This makes mesothelioma treatment convenient for many, with little travel required for doctor’s appointments, treatments, or hospital stays.
Others are not so fortunate. mesothelioma patients who live in rural areas or away from large cities may be required to travel in order to find the best care available for their disease. Most of these cancer centers understand that many patients travel quite a distance to take advantage of their services and they are eager to assist the patient and their family in making their visit(s) to the hospital as easy as possible.
Hence, these cancer centers employ helpful staff members that can assist with issues like long-term or short-term housing for patients and their families, especially for those who will remain in that location for treatment for an extended period of time. Most also provide other essential information for family members who will be staying with the patient including info on public transportation, dining, shopping, and more.
In most cases, these cancer centers also offer emotional and spiritual support for patients and families, including those nearby and those who’ve traveled many miles to take advantage of the top-notch services available at their institution.
National Comprehensive Cancer Centers
The National Comprehensive Cancer Network® (NCCN) has bestowed Comprehensive Cancer Center designation on those medical facilities that have demonstrated excellence in cancer care. Today, the NCCN alliance includes 21 of the nation’s best cancer institutes, working individually and together to eradicate cancer and promote programs that further cancer prevention. An estimated 160,000 new patients receive care at these member centers in any given year and many firsts have happened at these facilities through the years.
For example, NCCN member institutions were the first to devise the multi-disciplinary approach to cancer treatment, organizing teams of doctors from different specialties who would all work together to design treatment protocols for each individual patient that walked through the doors of their facility. In addition, NCCN offers guidelines for cancer care to non-member medical institutions who wish to duplicate their models for care.
Seeking treatment at one of these NCCN facilities means that patients will have the opportunity to consult with some of the best mesothelioma experts available and will receive cutting-edge care that includes the latest in treatments designed to fight asbestos-caused cancer. These centers are located across the country, mostly in major U.S. cities.
NCI-designated Cancer Centers
Since the 1970s, the National Cancer Institute, a division of the U.S. government’s National Institutes for Health, has been bestowing their designation upon those institutions that have been recognized for their scientific excellence; specifically, for their diagnosis and treatment of individuals with all types of cancer as well as for their development of more effective approaches to cancer prevention. NCI designation is voluntary; hospitals must apply for the designation.
The National Cancer Institute recognizes both Cancer Centers and Comprehensive Cancer Centers, the designation solely based on the type of grant received from the NCI. (There is no difference between the two as far as the quality of care offered.) Currently, there are 65 NCI-designated Cancer Centers throughout the U.S., including 58 that offer patient care and seven that conduct research only.
A visit to any NCI-designated center means patients will have access to some of the best mesothelioma doctors in the world, such as Dr. David Sugarbaker of Boston. These facilities also offer some of the most advanced treatment options available including mesothelioma brachytherapy, immunotherapy, gene therapy and mesothelioma photo-dynamic therapy. Many also conduct ongoing clinical trials that test new drugs or therapies for mesothelioma.
Cancer Clinics for Veterans
Because veterans - especially U.S. Navy vets - represent a large number of individuals who suffer from mesothelioma, there are numerous facilities that cater to their care. These facilities understand the needs of the veteran and his family and sympathize with the plight of the veteran who has been diagnosed with asbestos-caused cancer. In addition, these veteran-specific facilities can help vets determine what kind of services they are entitled to receive - often without charge - and also provide programs for spouses and other family members who need support through this difficult journey.

Malaysia Prudent enters talks with pension fund for 30% stake sale

;
British insurer Prudential’s wholly owned Malaysian unit has entered talks with KWAP, the country’s second-largest pension fund, for the sale of a 30% stake in the insurer.

Image may contain: 1 person, standing and indoor

The stake is valued at US$435 million and Prudential is reportedly in a hurry to sell it in order to meet the Malaysian government’s newly enforced foreign ownership limits.
However, talks between Prudential Assurance Malaysia Bhd and KWAP are not exclusive, and more concrete details could emerge within the month, sources told Reuters.
Malaysia’s central bank began enforcing a 70% limit on foreign ownership of insurers in order to promote domestic participation in the insurance industry. Aside from Prudential, several other foreign-owned firms in Malaysia such as Great Eastern (Singapore), Tokio Marine (Japan), and Zurich Insurance (Switzerland), have begun executing plans to divest before the deadline in June.
“Some insurers are hopeful that BNM (Bank Negara Malaysia) might extend the deadline but they realise that they still need to have a tentative deal in place,” a person familiar with the talks told Reuters.
Foreign insurers have been attracted to Malaysia and other Southeast Asian economies in recent years, due to strong economic growth, expanding middle classes, and low insurance penetration.

Wells Fargo faces Violations Over Car Insurance Refunds

;
The prospect of governmental action against Wells Fargo appears to be diminishing, despite mounting evidence of mistreatment of customers.

Wells Fargo, the scandal-plagued bank, is facing new regulatory scrutiny for not refunding insurance money owed to people who paid off their car loans early, according to people briefed on the inquiry.

Just last month Wells Fargo was found to have forced unneeded collision insurance on consumers who financed their car purchases. That practice, first disclosed by The New York Times, affected 800,000 customers according to an analysis commissioned by the bank. Some 274,000 people were pushed into delinquency as a result, and 25,000 cars were wrongly repossessed.
The latest inquiry, by officials at the Federal Reserve Bank of San Francisco, where the bank has its headquarters, involves a different, specialized type of insurance that is sold to consumers when they buy a car. Called guaranteed auto protection insurance, or GAP, it is intended to protect a lender against the fact that a car — the collateral for its loan — loses significant value the moment it is driven off the lot.
GAP insurance, also known as guaranteed asset protection, makes up that difference for a lender if, for instance, a car is stolen before the loan is paid off. Regular car insurance typically covers only the current market value.
Because Wells Fargo is a large auto lender, tens of thousands of customers may have been affected by the bank’s actions on GAP insurance.
It is not mandatory for car buyers to carry GAP insurance, which typically costs $400 to $600. But car dealers push the insurance, and lenders like it because of the protection it provides. When borrowers pay off the loans early, they are entitled to a refund of some of the GAP insurance premium because the coverage they paid for is no longer needed.
Laws in nine states require that customers get unused insurance money back. They are Alabama, Colorado, Indiana, Iowa, Maryland, Massachusetts, Oklahoma, Oregon and South Carolina.
Jennifer A. Temple, a Wells Fargo spokeswoman, provided a statement saying: “During an internal review, we discovered issues related to a lack of oversight and controls surrounding the administration of Guaranteed Asset Protection products. We are reviewing our practices and actively working with our dealers and have already begun making improvements to the GAP refund process. If we find customer impacts, we will make customers whole.”
Ms. Temple declined to say when the problem began. She said the bank was trying to assess how many customers had been affected. Wells Fargo improved controls on the refund process in 2014, she said. The unit of the bank that makes car loans is called Wells Fargo Dealer Services.
Asked about the regulatory inquiry into GAP insurance at Wells Fargo, Darren Gersh, a spokesman for the Federal Reserve Board in Washington said, “We are focused on ensuring that the root causes of a firm’s compliance and controls breakdowns are understood and addressed.” He declined to comment on the specifics, adding that “the Federal Reserve Board will take any regulatory and supervisory steps we feel are necessary to ensure the firm’s attention to compliance.”
A failure to refund the insurance money harmed borrowers whose cars were repossessed by increasing what they owed, a figure that the bank reports to consumer credit bureaus. All 50 states require that the amount of unused insurance be credited to those borrowers’ accounts, reducing the amount owed.

The bank alluded to the new problem briefly in its quarterly financial statement issued Friday. “The company has identified certain issues related to the unused portion of guaranteed auto protection waiver or insurance agreements between the dealer and, by assignment, the lender, which may result in refunds to customers in certain states,” Wells Fargo said in the filing.
“These and other issues related to the origination, servicing and/or collection of indirect consumer auto loans, including related insurance products, may subject the company to formal or informal inquiries, investigations or examinations from federal, state and/or local government agencies, and may also subject the company to litigation.”
GAP coverage is similar to home mortgage insurance, which shields lenders against a default if a borrower loses his or her job and cannot make the payments.
The new problem raises questions about Wells Fargo’s internal controls and its board’s oversight of company operations.
In a separate crisis at Wells Fargo that was exposed last year, bank employees were found to have created millions of credit card and bank accounts that customers had not requested. That led to millions of dollars in fines and the departure of the chief executive, John G. Stumpf.
More recently, after the disclosure that the bank had forced auto insurance on customers who did not need it, several Democratic lawmakers asked that hearings be convened to learn more.
Senator Elizabeth Warren, a Massachusetts Democrat who is on the Senate Banking Committee, also reiterated her request that the Fed oust 12 of Wells Fargo’s 15 directors, saying they had violated their duties to oversee risk management at the bank in the period when the improprieties had taken place.
In its regulatory filing on Friday, Wells said its directors had undertaken actions to enhance governance and oversight.
“The board recognizes that there is still work to be done, and, in response to feedback received at our annual stockholders meeting in April 2017, the board is engaging in an ongoing comprehensive review of its structure, composition and practices,” it said. The bank expects the review to result in changes to be disclosed in the coming months, it added.

In a statement to employees issued Friday with the filing, Timothy J. Sloan, Wells Fargo’s chief, said, “To regain the trust we have lost, we must continue to be transparent with all our stakeholders and go beyond what has been asked of us by our regulators by reviewing all of our operations — leaving no stone unturned — so we can be confident we have done all that we can do to build a better, stronger Wells Fargo.”

Medium and Small Businesses Without Insurance Take Dangerous Risks, see how here.

;
Often, small businesses and startups work with a tight budget. As a result, it might be tempting to forego certain types of insurance that aren't required by law. In fact, a survey of 30,000 business owners conducted by Next Insurance found that 44 percent of responding businesses have never had insurance at all.

Image may contain: one or more people, people standing and outdoor
Unfortunately, taking such a risk could end up costing way more than the monthly premiums ever could. Jamie Dokovna, an employment attorney with Becker and Poliakoff, said failing to purchase insurance simply because it isn't legally required isn't necessarily the best move. For instance, if a customer slips and falls on your property, injuring themselves in the process, it typically costs up to $20,000 to settle a subsequent lawsuit.
"It's often a case of being pennywise but pound foolish," said Dokovna. "For some small businesses, they look at the cost [of insurance] and they say, 'Well, it's a little expensive, so I'm willing to take the risk.' But it's not cheaper to forego insurance when you need it and wish you'd had it."
Instead, Dokovna said, it's best to make strategic decisions about which insurance policies your business needs and which it doesn't. The only way to do so is to know your industry well.
Some kinds of insurance coverage are required by law if your business reaches a certain size. For example, the Affordable Care Act mandated employer-sponsored healthcare coverage for businesses with 50 or more employees. Similarly, most states have a workers' compensation requirement, which requires employers that reach a certain employee threshold (it varies from state to state) to maintain workers' compensation insurance and policies. Failing to retain these types of insurance doesn't just expose small businesses to the risks they're intended to cover but also to governmental action for violating the law.
There are other types of coverage that aren't required by law but might be wise to have depending on your line of business. In keeping with the slip-and-fall example above, fees for attorneys and settlements could be covered by professional liability insurance. However, only 21 percent of businesses report maintaining this type of coverage, meaning the vast majority are vulnerable.
That said, certain businesses might deem it a remote risk that someone visits their property at all, let alone injures themselves, so they forego coverage. This is a strategic choice, and likely an acceptable risk, as opposed to a retail store that sees customers daily and chooses to roll the dice anyway, for example.
There are many types of business insurance, so before you shop around for a plan, it's important to consider your specific needs. A general liability, or business owner's policy, is good for umbrella coverage, but depending on the type of business you own, there may be other insurance policies that better protect your business. Jeff Kear, owner of event-management software Planning Pod, said that work-from-home business owners should consider separate home-based business insurance.
"Don't assume that your homeowner's policy will cover your business assets, because many policies do not cover most home-based business losses," said Kear. "They may not cover all assets, and probably won't cover any kind of business or professional liability."
Kear also recommended obtaining business-interruption insurance to help keep your business afloat in the event of natural disasters, data loss or theft.
Choosing an insurance provider is like any other major decision: You should always consider all options before making a final choice. Comparing quotes from multiple providers can help you get the most comprehensive coverage for the best price.
"Get at least three quotes from insurance providers, and do a comparison based on the deductible, the premium, what damages are and are not covered, and the quality of customer service," said Andrew Schrage, founder and CEO of financial advice site Money Crashers.
The insurance agent or broker you work with is the person responsible for helping you protect your business. As with the plan itself, consider your options for agencies, and don't necessarily choose the one that's closest to you.
"Look for an agent who specializes in business insurance and can be a long-term partner," said Mike Wolfe, co-founder and CEO of marketing agency WAM Enterprises. "It's important to establish a relationship with your agent. Research online, and ask other business owners who they work with. We have several agencies in our town, but [we] decided to do business with an agent who is farther away, because we developed a relationship and trust."
An insurance broker, rather than an agent who works for a specific provider, may be a good choice for business owners who want to find coverage from different providers to suit all their needs, said Kear.
"When you're sitting down and talking with an agent, make sure the agent understands your business and what you do, and how many employees you have. Most good agents will know what you need," Dokovna said. "Some small businesses even use [professional employer organizations], and so those companies sort of act like a super employer; they offer other types of insurances which are often cheaper and more cost-effective for a small business owner being able to tap into a larger resource pool than they would have on their own."
Most insurance policies need to be renewed annually. Before you sign on for another year of coverage, it's wise to look over the fine print of your policy and account for any changes, either in your business or in the provider's terms of service.
"Coverage and policies change all the time, so review your business with your agent every year," said Paige Dawson, founder and president of marketing firm MPD Ventures. "Your business may have changed during a coverage year, and [your policy] may no longer be adequate. Adding or dropping employees, services, products, physical locations, etc. can have an impact on your policy."
If your business goes through a major change or transition in the middle of coverage, discuss it with your insurance agent as soon as possible, and have him or her walk you through your options. Depending on the change, you may even be able to save money on your policy.
